Online Entry Form for ADCS Festival of Driving Pleasure Show
October 29-30, 2022

This Form is best viewed and filled out via a computer as opposed to a smart phone. Use this Form to enter this Event and Pay your fees. Online Early Entry deadline is Oct. 13th. Late Entry deadline is Oct. 21. No NEW entries will be accepted after this date or onsite show days, but those pre-entered may add classes at the show.
    
Use a Separate Entry Form for Each Turnout (Driver, Equine, Carriage Combination). This form saves your progress, and will reset after a submission.

 

Payments are processed through PayPal, and non-refundable Transaction and Processing fees apply. Download the Entry Form.pdf and mail it with a check to avoid paying these fees. All fees shown below have been adjusted to include their portion of the transaction fee, and the processing fee has been included as well, in the Office Fee.

 

Note: After submitting the form, you should be redirected to PayPal to process your payment. Some folks have a problem with this, probably due to pop-up blockers, which can be removed in your browser preferences. If you are not redirected to PayPal, we will receive your entry, but no payment. If your payment goes through, you will receive a receipt from PayPal. If you have problems with this form or the payment, please contact the webmaster.
     

If the submit bar on the form shows a price with more than two decimal places, just round that total when you enter it on the "Grand Total" line. (unsolved app glitch)


IF ENTERING ONLINE: Submit the following-

  1. the Online Entry Form below

  2. the ADS Disclaimer/Hold Harmless Agreement (Download this form to your computer - use the download icon that appears if you hover over the bottom of the form, or use "export as pdf" from the file menu. The form that pops up when you click on the link above does not allow a signature, but the downloaded version does. Fill out and sign the form, save it with your last name in the file name with "waiver" and upload it at the bottom of the entry form. NOTE: All in your party and horse owner need to sign this waiver. If not available to sign on your waiver, direct them to this waiver on the Event webpage for a separate online submission or to print off and turn in at event, or they need to sign one at the event at check-in. OR if you have a problem with this form, try using this link to another version of this form which can more easily be signed online and submitted directly.

  3. If an ADS member, give your ADS # on the entry form, or submit the Affidavit. (This form is not fillable unless you open it in a program that allows fill and sign. But you could print it off, fill it out, and bring it with you. If you are able to fill and sign on your computer, then save with your last name and "affidavit" in the file name and upload it on the entry form.)  OR

  4. If not an ADS member, download the Non-Member Credit Voucher to your computer, fill it out and sign, save it with your last name and "voucher" in the file name, and upload it below on the entry form. (This form is fillable and can be signed if downloaded first; do not try to use the form that pops up when you just click the link as it does not allow signing.)

  5. Other documents that are required, like coggins, vaccination record, health certificate, pony card, etc. can be brought with you for presentation at check in, or, if you have them in digital form, can be submitted via email to the entry secretary: Karen Austermiller
